New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. All newly registered patients are requested to have a registration medical within three months of joining the practice, even if you have already seen a doctor for another reason. Please make an appointment at reception for this. We feel this is very important in order to introduce ourselves and get background details of your medical history.
Medical treatment is available from the date of registration. Please contact reception for further information.
Named GP’s. All patients are automatically registered with Severn Fields Medical Practice. As part of a government initiative patients have also now been allocated to a named accountable GP. It is important to note that patients are still free to see whichever GP they choose however any incoming post of results will be reviewed by your named GP and will have overall responsibility for your care. If you would like to know who your allocated usual GP is then please speak to any member of staff when you next attend the practice or alternatively ask at receptionist.
